Group ID and Benefit Number are Required on all Humana MA/MAPD/PDP Paper Applications

The Group ID and Benefit Number are required on all MA/MAPD/PDP paper applications. Without this information, Humana’s enrollment team may not be able to determine which plan to enroll the member in. Additionally, missing Group ID/Benefit Number are among Humana’s top incomplete errors every year and may lead to enrollment denial.

There are two ways agents can find the Group ID/Benefit Number:

  1. Locate it on the attached grid (Group ID = Column D; Benefit Number = Column E)
  2. Click on the link below and a map will appear. Click on the state and you’ll see a list for the plans in the state. The Group ID and Benefit Number (also called the BSN) will be displayed.
